Terms of Use
These Terms of Use (“Terms”) govern your use of the app “Myopia Log” (the “App”) provided by MonopoleApps Inc. (“we,” “us,” or “our”). By installing or using the App, you (“User”) agree to these Terms.
The App provides a Clinic mode (a recording/viewing tool for healthcare facilities) and a Patient mode (for patients/guardians to view their own measurements).
Neither mode is a medical device for diagnosis or treatment. All medical decisions must be made by licensed professionals. Do not use the App for emergencies, self-diagnosis, or self-treatment.
1. Purpose and Nature of the App
The App is a record-management viewer to store, view, and share values measured by clinical devices (e.g., axial length). It is not intended to present diagnoses, determine treatment plans, or evaluate/predict conditions. It is not certified/approved as medical device software.
2. Target Users and Modes
- Clinic mode: Clinic staff (physicians, nurses, technicians, clerks, etc.) can record/view patient data under the clinic’s account.
- Patient mode: Patients/guardians can view their own measurements via links/QR codes issued by clinics. The visible scope depends on the clinic’s settings.
3. Accounts and Linking
Users may create accounts using Apple/Google sign-in. To maintain links across device changes or re-installs, we may associate the user ID with linking identifiers (e.g., personId). Patient-mode linking is activated via time-limited links/QRs issued by clinics upon user receipt/consent. You may request unlinking in-app or at the clinic desk.
4. Information Collected and Stored
The App may store/display the following (see also Privacy Policy):
- Measurements (e.g., left/right axial length, measurement dates)
- Patient identifiers (facility ID, name, DOB, etc.; may be encrypted/tokenized as needed)
- Linking/access logs (linking time, devices used, etc.)
5. Rights and Data Handling
- Original medical records recorded at clinics are governed by each clinic’s policy. Patient-mode viewing is limited to the scope authorized by the clinic.
- Users shall use the App in compliance with applicable laws, guidelines, and clinic policies.
6. Medical Disclaimer
Content shown in the App is for recording/viewing assistance and is not guaranteed to be perfectly current, complete, or accurate. Do not rely solely on the App for diagnosis, treatment, or instructions. To the extent permitted by law, we are not liable for damages arising from use or inability to use the App.
7. Prohibited Conduct
- Use beyond the intended purpose (recording/viewing measurements)
- Use for self-diagnosis, self-treatment, or emergency decisions
- Handling of personal/medical data in violation of laws, guidelines, or clinic policies
- Unauthorized access, reverse engineering, modification, copying, redistribution
- Impersonating a third party to log in
8. Fees
Some features may require paid plans or subscriptions. Pricing/terms are as shown in the App or store listing.
9. Changes and Suspension of Service
We may add/change/suspend/terminate features as necessary. We will provide advance notice of important changes when feasible; urgent cases may proceed without prior notice.
10. Governing Law and Jurisdiction
These Terms are governed by the laws of Japan. The court with jurisdiction over our head office shall be the exclusive court of first instance.
11. Amendments
We may revise these Terms. Revisions take effect when posted in the App or on our website.
